Chronic condition management

Transform your health

Managing a chronic condition can be complex and frustrating. Forty-five percent of all Americans have a chronic condition — and that number is growing.

If you’ve been diagnosed with one of six health conditions, our chronic condition management program will help you set goals and give you help managing your condition. Our program focuses on:

  • Diabetes (adult and youth)
  • Asthma (adult and youth)
  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease (COPD)
  • Coronary artery disease
  • Renal disease

Once you're enrolled, you'll be assigned a case manager who will be your primary contact throughout your time in the program. Download a flyer to learn more.

What will I get?

Chronic condition management participants will get:

  • Scheduled calls with a case manager
  • Educational materials for self-management skills
  • Toll-free number for support when you don't have a scheduled call
  • Help locating community resources for support, medication assistance, etc.

The program is covered at no cost to members, which means you won't pay anything to participate.

Who gets to participate?

Have you been diagnosed with one of the six conditions? Call the toll-free number on the back of your member ID card and tell them you're interested in learning more about the program. Your information will be reviewed by a case manager, who may then contact you about joining the program.*
