Principal’s Endorsement
Applicant hereby certifies that the principal of your School is aware of and has agreed to the creation of a Calming Room at your School (if funded through this award) and has appointed an employee of your School to oversee/manage the Calming Room activities.
Completion of Project
Applicant hereby agrees to the completion of the creation of the Calming Room by May 1, 2024.
Disclaimer
The Take Good Care Calming Room funding Award (Award) is provided by ABCBS. Applicants must be employed by the School which is the applicant as a condition of the Award. Applicant Representative must be at least twenty-one (21) years of age at the time the Application is submitted. ABCBS reserves the right, in its sole and separate discretion, to disqualify any Application for any reason or for no reason, ABCBS reserves the right to terminate, modify, withdraw, or suspend Award at any time for any reason or for no reason. Application period begins at 12 a.m. on August 1, 2023, and ends at 11:59 p.m. on September 22, 2023.
Award will be available to all private, public and charter schools who meet the required Application Guidelines for entry for the chance to receive one of 75 $2,500 cash Awards for the sole purpose of creating a Take Good Care Calming Room. ABCBS employees, officers or directors are not eligible to participate. Winners will be contacted via phone or email. All taxes, if applicable, on any Award are the sole responsibility of the recipient. Each applicant/school releases and agrees to indemnify and hold harmless the Company, their respective partners, affiliates, subsidiaries, related companies, directors, officers, employees, representatives, agents, successors and assigns from all liability for any injuries, loss, or damage of any kind to applicants/recipients/school arising in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of Award. School shall bear all risk of loss or damage to the Award after it has been accepted and/or completed. No purchase is necessary to apply for this Award.

Photo/Video/Media Release Consent

Consent to Photo/Video/Media Release:
You consent and authorize ABCBS to use Your likeness in any interviews, photographs, video or audio tape or other digital media (Photographs) pertaining to the creation and completion of the Take Good Care Calming Room in any and all ABCBS publications, print, online, web-based and advertisements of all kinds in all media (Publications).
You authorize ABCBS to copy, enhance, edit, crop or otherwise alter any Photographs for use in Publications. You also waive any right of approval or inspection of any Photographs.
You acknowledge that all Photographs are and remain the property of ABCBS and will not be returned to You. On behalf of School, You acknowledge that You are not entitled to any royalties or compensation in connection with the use of the Photographs.
As an authorized School representative, You agree to secure acknowledgements and permissions from other School staff/personnel for the use of Photographs of their likeness related to promoting the Take Good Care Calming Room as needed.
As an authorized School representative, You agree to secure acknowledgements and permissions from parents or guardians for the use of Photographs of persons under the age of 18 as needed.
