Your Tax Forms
We were unable to find a 1095-B form for your policy. This may be because your form is not yet available for the year. Forms typically generate in February each year. You should have a 1095-B form available from Arkansas Blue Cross if:
- You bought your coverage directly from us and didn't go through healthcare.gov.
- You received your coverage through your employer and it met the healthcare reform standards for minimum essential coverage (MEC).
Please note: The Affordable Care Act requires fully insured large employers (50+ employees) and employers that are self-funded to send their employees 1095-C forms. If you work for a self-funded employer, you will only receive a 1095-C from your employer. Talk to your employer’s human resources office if you have questions about which form(s) you may receive.
