Order Certificate of Creditable Coverage
A Certificate of Creditable Coverage provides evidence of your medical insurance coverage. If you become eligible for another group insurance plan that excludes coverage for certain pre-existing conditions, this certificate may enable you to decrease or eliminate your pre-existing condition exclusion period providing that you do not have more than a 63-day break in coverage. A condition is considered pre-existing if medical advice, diagnosis, care, or treatment was recommended or received for a condition (mental or physical) within a 6-month period ending on the effective date of full-time employment if a new employee, or the effective date of coverage on your new group insurance plan. If you become eligible for another group insurance plan, check with the plan administrator to see if you need to provide a copy of this Certificate of Coverage. You may also need this certificate to buy, for yourself or members of your family, an insurance policy that does not exclude coverage for conditions that are present before you enroll.
You should receive your Certificate of Creditable Coverage letter within 5-7 business days via the U.S. Mail. A Certificate of Creditable Coverage letter must be ordered for each family member.
