that relate to HealthConnect Blue, a service for eligible members.
- Clinical Evidence (278 KB PDF): The HealthConnect Blue Program provides objective, information-based evidence from nationally recognized sources, such as National Institutes of Health (NIH), Centers for Disease Control (CDC), American Diabetes Association (ADA), American College of Cardiology (ACC)/American Heart Association (AHA). Clinical evidence is updated annually and as new, relevant clinical information becomes available.
- Effective Care Insights (203 KB PDF): The Effective Care Insights are brief summaries of the evidence for each chronic condition, highlighting the areas that HealthConnect Blue Health Coaches may discuss with your patients.
