This program has recently changed to a more robust case manager-driven education program that you may be interested in. This means that a licensed, nurse case manager will be directly involved in your health education. We believe this new approach will be more beneficial because you can work directly with a case manager who will streamline and choose educational materials to best meet your individualized goals.
Changing to an individualized educational experience will allow us to be more comprehensive in a single mailing, give you more focused educational materials, and allow you more time to engage with a nurse if you choose. This is all part of your employer's benefit plan and is no cost to you.
Please note that you must speak to a case manager to be enrolled in the new program, so if you wish to participate (and we hope that you do!), please call us at 800-225-1891.
