DentalBlue Benefit Certificates

Elective Dental Coverage (Stand Alone) (135 KB PDF): This plan is paid/sponsored by the employer, and the coverage type is independent of medical coverage.
Endorsed Dental Coverage (117 KB PDF): This plan is paid/sponsored by the employer, and the coverage type is tied to medical coverage.
Voluntary Dental Coverage (142 KB PDF): This plan is paid by the employee.
