Job Summary
This position is responsible for low risk members enrolled in the Life with Baby Pregnancy Program. The incumbent completes all 2nd risk assessments for low risk members enrolled in the program between 24-28 weeks gestation. The incumbent also completes all outcomes assessments for low risk enrollees after delivery. All assessments must be completed in a timely manner

Nature & Scope
The incumbent is responsible for the implementation of the program for all low risk members including phone calls to members, mailings, documentation and data entry. The incumbent is responsible for providing required information for weekly, monthly and quarterly reporting in a timely manner.

Minimum Job Requirements
Minimum Job Requirements 1. Current LPN license in the State of Arkansas 2. Clinical experience 3 Experience in obstetrics preferred. 4, Keyboarding skills. 5. Ability to use a PC with software (i.e., Word and Excel) 6. Above average telecommunication skills

Segregation of duties will be used to ensure that errors or irregularities are prevented

PRINCIPAL ACTIVITIES OR ACCOUNTABILITIES (Essential Functions of Job)
Job Responsibilities:
1. Generate reports from LWB system as needed to maintain 2nd risk assessments and outcomes for low risk members.
2. Document all outcomes information received from incoming mail for high risk and low risk members.
3. Make 2 outbound calls to low risk members to complete 2nd risk assessments and outcomes. Document findings.
4. Mail 2nd risk assessments and outcomes surveys to members unable to reach by telephone.
5. Notify RN team member of any problems or unusual findings from all communications with members, phone or mail.
6. Provide reporting information as requested by management team.
7. Maintain positive working relationships with other employees in a team environment.
8. Continuously improve work skills and knowledge by participating in employer sponsored education/training programs.
9. Participate in additional external training and education programs.
10. Evaluate process and create plans for improvement.
11. Complete other duties as requested.
12. Maintain and assure complete confidentiality in all aspects of job related information.
