Child-Only Applications Submitted as the Result of a Qualifying Life Event

Child-Only major medical applications, for individuals age 18 or younger, may be submitted throughout the year only as a result of one of the following "Qualifying Life Event (PDF)" changes:

• Loss of employer-sponsored health insurance if the employer has ceased providing health insurance or the insured has lost health insurance with the employer because of termination of employment for any reason other than fraud, misrepresentation, or failure to pay premium.
• "Involuntary" loss of other existing health insurance.

In such cases, the applications must be submitted within 30 days of loss of coverage. Documentation of prior health insurance will be required.
