• Basic Restorative
• Major Restorative
• Orthodontia

Groups with DentalBlue coverage may choose from the Preferred Payment Plan (PPP) network of more than 850 participating providers (dentists). (Go to Provider Directory for a complete list.)

DentalBlue Benefit Certificates

• Elective Dental Coverage (Stand Alone) (135 KB PDF): This plan is paid/sponsored by the employer, and the coverage type is independent of medical coverage.
  
• Endorsed Dental Coverage (117 KB PDF): This plan is paid/sponsored by the employer, and the coverage type is tied to medical coverage.
  
• Voluntary Dental Coverage (142 KB PDF): This plan is paid by the employee.
