Enrollment Limitations
Medicare beneficiaries may enroll in a plan only during specific times of the
year. For more information, see Enrollment Guidelines or call us at 1-800-634-6314, 8 a.m. – 8 p.m., 7 days a week (TTY: 1-800-370-5869).
Benefit Changes
Medi-Pak Rx benefits, formulary, pharmacy, network premium and/or copayments/coinsurance may change on January 1, 2009. Please contact Medi-Pak Rx for details.
Network Limitations
Eligible beneficiaries must use network pharmacies to access their prescription drug benefit, except under non-routine circumstances when they cannot reasonably use network pharmacies.
